Online HMO Approval

In our efforts to make our patients’ HMO approval more efficient, please feel free to use this form to lessen your waiting time at the hospital. We provide online assistance in scheduling appointments and LOA coordination with your providers for onsite outpatient utilization of our services.

LOAs are advised to be filed at least a days prior to your availment date. Please allow us at least 24 hours to process your request once submitted.

Please make sure you get the requested tests done within 3 calendar days after approval as the amount involved may be automatically deducted from your insured limit. Please be informed that we still need to call your health insurer for approval. Wait for our e-mail or text confirmation before proceeding to the hospital to avoid any inconvenience.

We are open Mondays to Saturdays from 8:00 AM-05:00 PM (Cut-off of registration at 04:00 PM).

Registrations made on Sundays will be entertained on Monday.
