For Philhealth:

For registration, updating/amendments (like declaration of beneficiary, discrepancy on the members/dependents details)

1.  Properly accomplished PMRF (philhealth member registration form)
2. Birth certificate of patient/member.
3. Marriage contract of patient/member.
4. Valid ID of patient/member.
5. Other supporting documents deem necessary.

For Billing

1. Daily progress Billing.
2. Discharge cut off: HMO category: 4:00 pm
   Philhealth/Self Pay: 5:00 pm

• Tru Check via G.L
• Credit Card
• Tru Bank Transfer
